Download and print the RX form and any additional preference forms. For additional supplies, request supplies here.

2. Schedule your case

Write the requested return date on the RX. In the case of any concerns, or questions, 

please call 201-468-3396.

3. Call for case pick-up

If local, call our toll-free line at 201-468-3396 to schedule a pick-up.

4. Case packing and shipping

If not local, please refer below to Required 

Case Materials, Cosmetic Case Checklist, 

and Shipment Preparation guidelines. 

Download our UPS labels and attach them to your package or call us for a pick-up.

5. Track your case

Create a doctor portal account to view, monitor, and track your case anytime and anywhere.
